CARES (Community Assistance, Referral and Enrollment Services) is a holistic social advocacy program designed to assist Medicaid beneficiaries to gain a greater understanding of the public or private benefits and services that are available to them. Since inception, SSC has worked with health plan members to help improve their quality of life and have a positive effect on their overall health and well-being. With CARES we can help bring much needed relief from some of the financial challenges facing health plan members.
SSC has developed CARES in response to our MA plan customers with Managed Medicaid plans asking SSC to extend our expertise and services to their Medicaid Members. Our expanded services help Managed Medicaid Plans with their retention and recertification efforts. Providing members with recertification reminders and access to our advocates to help guide them through the renewal makes a positive impact on retention. This engagement effort will cover the span of the calendar year as oppose to most existing state and plan efforts of just one quarter.
CARES is like a “virtual” community-based organization that helps consumers retain their coverage and access other free social and community programs for which they are eligible and which support the overall health and wellbeing of the family.
For example, a member that is connected and enrolls in programs that can lower their cost on fresh produce and utilities can help a family to eat better, stay warm and communicate with their family, health plan, physicians, and school. These connections go beyond what a health plan can do to help a family in need to maintain a happy and healthy home.
CARES is offered as co-branded service or can be private labeled so that the assistance increases brand awareness and loyalty.
SSC has developed an extensive database of over 7,500 public and privately sponsored social programs in all 50 states. With a single interview, the social program database and SSC’s proprietary eligibility evaluation tool enables our advocates to determine the programs and services available in a plan member’s local area. From there the appropriate applications are completed electronically for submission to the program sponsor.
A CARES outreach campaign can simultaneously reduce churn, increase participation in other initiatives and build plan loyalty from members. This provides an efficient outreach process for the Health Plan and a rewarding experience for the member. When our services have been offered to the Age, Blind and Disabled (ABD) Medicaid and Medicare population our experience shows consistent, substantial increases in the retention of members.
